A browser app for real-time cognition and behaviour recording

Website that can function as an app, for real time cognition and behaviour recording at two linked time points. By using a website, we hope to get around Iphone/Android and software/operating system update requirements.
Time 1: Individuals complete an initial brief questionnaire. This could be triggered by:
User input (e.g. participant logging in to report event)
Push notification at a pre-specified time
Push notification at a random time within a pre-specified time window
Time 2: Individuals complete a second brief questionnaire. This could be triggered by:
Pre-specified time following user input at Time 1 (e.g. 1 hour after initial input)
Random time within a pre-specified time following user input at Time 1
Content needs to be modifiable by research team, in terms of:
Topic, content and number of questions at both time points
What triggers the push notification, and length of time between push Time 1 and 2
Examples:
Individual records urge to consume alcohol. Two hours later, they are prompted to record how much alcohol they consumed
Individual records intrusive thought. 30 minutes later, they are prompted to record how long the thought occurred for, and how distressing they found the thought
4 times a day, an individual is prompted to record their current level of pain, and their current activity
At 11am, an individual is prompted to record their distress and current activity, and between 30 and 60 minutes later, they are prompted to record their distress and current activity again.
